Parents and guardians should submit information for minors. Please do not include birth dates, home addresses, school schedules, medical information or other sensitive personal details. Team Infinity only needs enough information to understand the athlete's basketball interest and respond. Submissions are processed by FormSubmit for email delivery and may be retained for up to 30 days.
